Engaging patients post-hospitalization The T400 Telehealth System includes the T400 touch-screen device, the web-based T400 Desktop application for providers and over 25 clinical content programs for a variety of single, co-morbid, and tri-morbid medical conditions. It is used for patients who are post-discharge from the hospital and in the transitional period after this. How the T400 Telehealth System works Every day, the patient enters their vital signs into the T400 device. The patient is then asked to answer a series of pre-defined questions and may be provided with educational information. The information from the vital signs is then transmitted to the health provider for review as individual and trended data. Additional measures of vital signs may also be taken each day and sent to the healthcare provider. The T400 device is able to gather vital sign data from a variety of peripheral devices, either through self-reporting or through wired connectivity.
