Designing a Hospital Cardiology Outreach Service

The cardiology team at Sheffield Teaching Hospitals NHS Foundation Trust successfully designed and implemented an outreach service for heart failure patients in non-cardiology wards (National Institute for Health and Care Excellence 2014a).
