Portavita focuses entirely on care for people with chronic conditions. Our web-based healthcare chain information system KIS (Keten Informatie Systeem) contributes to improvement of this type of care by providing support for multidisciplinary, integral treatment. The patients themselves can if so desired make an active contribution, because patient access is an integral part of the KIS. Each module in the KIS focuses on a different condition. The content of these modules is largely determined by physicians who are leaders in their field of expertise, and by the chronic patients themselves.
