Care Indeed is looking for enthusiastic, detail-oriented, and skilled professionals with a can-do attitude to join our team as full-time Care Managers. As a member of our team, you will communicate with clients and their families, doctors, and medical staff; and monitor clients' situations on an ongoing basis to ensure that changing needs are addressed. Strong management, leadership, and problem-solving skills and a willingness to learn and continuously improve are essential for success in this role.

A demonstrated background in education skills, knowledge, and experience in aging and care

management is also necessary to provide quality care management services.

DUTIES & RESPONSIBILITIES:

  • Update client records, manage changes in care plans and conduct reassessments.
  • Ensure successful staffing and Coordination of care
  • Collaborate with supervisors to meet clients’ care needs.
  • Assist in quality assurance visits and check-ins.
  • Provide resources and training to caregivers.
  • Accompany clients to appointments when required.
  • Document family or caregiver reports regarding client status for the Care Managers and respective family point of contact.
  • Assist in coordinating client care conferences, caregiver orientation, and training.
  • Monitor assigned accounts and assist in reports and data entry.
  • Provide additional support for new client cases, including home visits during the initial care visits, and serve as a backup when needed.
  • Establish and maintain referral resources to ensure referral sources' satisfaction.
  • Execute additional duties as required to support the Client Care Managers.

QUALIFICATIONS & EXPERIENCE

  • A Bachelor's degree in psychology, counseling, rehabilitation, gerontology, nursing, or sociology is required.
  • Specialized training in gerontology/geriatrics is preferred.
  • A minimum of 3 years of healthcare experience, with prior client management and/or marketing experience in the health/home care industry being a plus.
  • Strong presentation skills with the ability to effectively communicate the value of Care Indeed and its mission.
  • Proficiency in Microsoft Office (Word, Excel, PowerPoint, and Outlook).
  • Availability during and after business hours, and for on-call rotation on weekends.
  • Excellent written and verbal communication skills with an eagerness to learn.
  • Unwavering commitment to ethical work standards in all interactions with clients and employees.

SALARY

  • $70,000 - $80,000 Annually

  • Schedule: Monday - Friday, 9:00am - 5:30pm

This job operates in a professional office environment. This role routinely uses standard office equipment such as laptops, smartphones, photocopiers/scanners, shredders, and filing cabinets. Risk of exposure to bloodborne pathogens-limited

  • The position is stressful in terms of meeting deadlines.
  • It requires minimal lifting of office records and printouts.
  • It is primarily a desk job that essentially involves sitting, standing, stooping, and walking, as well as an inordinate amount of telephone communication
  • Travel is required; use of personal car to local, out-of-town seminars, conferences, or meetings
