Job Overview

An exciting opportunity has arisen for an experienced Advanced Clinical Practitioner to join the newly established Community Complex Conditions Service in North Wales (base to be agreed upon appointment).

This is a professional clinical and team lead role the job plan focus being clinical activity and the balance being clinical leadership, research, audit, and education and developing the clinical models. Developing clinical services across Community Complex Conditions through working in partnership with the consultant therapist, nurses, therapists from other disciplines and their professional leads.

The post holder will as an autonomous, advanced practitioner, with an extended scope of practice to work with the team to deliver care within the relevant specialism; providing a high standard of specialist assessment and treatment of patients. Using evidence-based principles/ current best practice and advanced specialist skills and clinical reasoning to assess, plan, implement and evaluate interventions.

Main duties of the job

We are looking for someone who enjoys working collaboratively as part of a skilled inter-disciplinary team. The team includes Clinical Psychology, Assistant Psychologists, Advanced Clinical Practitioners, Physiotherapy, Occupational Therapy, Dietetics, GP and support staff. The service supports people presenting with Long Covid, other post viral and fatiguing conditions, persistent physical symptoms, breathing pattern disorder and those who present frequently to services. The new Community Complex Conditions Service provides an opportunity to continue development of this service and shape the way we work with vulnerable groups who might otherwise fall through the gaps in services. We are an award winning, friendly and supportive team; who are research active, with a passion for co-production with our partners with Lived Experience.

This post would be suitable for applicants from a variety of professional backgrounds depending upon experience and qualifications. Both full and part time hours will be considered.

Qualifications

Essential criteria

  • Degree or recognised equivalent in nursing, para medicine or an allied health profession
  • Masters level education in relevant discipline
  • Active NMC or HCPC professional registration
  • Evidence of CPD and post registration activity through portfolio
  • Extended practice/Extended scope/Expert level skills
  • In depth understanding of psycho-social interventions, normal development, the social determinates of health and the social disability model

Desirable criteria

  • Membership of clinical interest group
  • Membership of professional body
  • Practice placement educator qualification/Clinical leadership training
  • Non-medical prescribing

Experience

Essential criteria

  • Extensive experience in clinical field
  • Experience of designing, delivering and evaluating groupwork.
  • Significant post graduate clinical experience in the NHS
  • Experience of management of people/resources

Desirable criteria

  • Formal research education

Aptitude And Abilities

Essential criteria

  • Advanced clinical reasoning skills
  • Able to deliver assessments and interventions within physical, psychological, social and learning needs scope of practice.
  • Able to desgin, select administer and analyse assessment tools and information to identify intervention goals.
  • Excellent skills oral and written communication skills
  • Ability to critically appraise current research/evidence and implement relevant findings.
  • Ability to conduct professional affairs in a confident, assertive manner, using negotiating/influencing skills whilst maintaining excellent working relationship with all stakeholders.
  • Health and safety risk awareness. Knowledge of clinical governance
