Our ultimate goal is to get to ZERO preventable patient deaths by 2030!
- Prioritizing safety with patients at the center
- Promoting dignity, compassion and respect in healthcare
- Aligning everyone who influence or is touched by healthcare
- Promoting transparency
- Realigning incentives to achieve safer care
- Informing caregiving, patients and families about patient safety
- Identifying the preventable causes of harm and death, and then create and freely share actionable solutions (Actionable Patient Safety Solutions) to mitigate them.
- Driving the implementation of Actionable Patient Safety Solutions (APSS)
- Asking healthcare technology companies to share data to create a Patient Data Super Highway

People are still dying in hospitals, every day, in ways that could have been prevented. ZERO preventable deaths from medical error are possible. Rather than targeting population- or disease-specific performance improvement initiatives, true progress demands building a solid foundation of safe and highly reliable care, which has to include:
- A Person-Centered Culture of Safety
- A Holistic, Continuous Improvement Framework
- An Effective Model for Sustainment
We believe that, without a commitment to these three critical components, ZERO cannot be achieved. Here at the PSMF, we want to help healthcare organizations make this happen by expanding our Actionable Patient Safety Solutions (APSS) to include APSS Blueprints, APSS Education, and APSS Coaching, which complement one another to deliver multifaceted education and performance improvement tools to meet the needs of the user. Together, these tools are designed to help healthcare organizations meaningfully integrate best practices for improvement into their processes in a sustainable manner.
The Patient Safety Movement Foundation is not just about information, it is about action! The PSMF has developed 50+ leading patient safety challenges that healthcare organizations commonly face around the world.

The Patient Safety Movement Foundation wants to create more highly reliable systems! APSS coaching is available for FREE to healthcare organizations who make a commitment to ZERO preventable death and harm and to others for a nominal fee. We provide virtual coaching and consulting on specific desired performance improvement projects. In addition to coaching based on your organization’s immediate need, we will work to develop your team members’ capacity to embody a performance improvement mindset so that you can sustain and grow improvement as a team even after we leave
