Opening Keynote Address: Restructuring Care Teams to Achieve the Quadruple Aim

The implementation of value-based care models adds to existing demands on providers to demonstrate increased clinical productivity and financial efficiency. Reliant Medical Group has initiated changes across its organization that support clinical staff in improving care quality -- and at the same time, mitigate the myriad factors contributing to provider burnout. President & CEO Tarek Elsawy, MD, will detail how Reliant's novel approach to team-based care is revolutionizing care delivery for both its providers and patients.

Tarek Elsawy, MD, FACP, President & CEO, Reliant Medical Group

Fireside Chat: Translating Big Data into Actionable Business Intelligence

Healthcare organizations don’t need data. They need actionable insights that can measurably improve patient care. But how can providers transition from collecting big data to leveraging it effectively? Learn how to develop a big data analytics roadmap that will incorporate new strategies like machine learning and predictive analytics to bring meaningful results to the bedside.

Joe Kimura, MD, MPH, Chief Medical Officer, Atrius Health

Panel Discussion: Implementing Practice Transformation Strategies for Population Health Management

Value-based care demands that healthcare organizations take a new approach to managing patients and meeting both their clinical and administrative needs. As risk-based contracting starts to offer attractive financial rewards for embracing population health management, how can providers alter their daily workflows to hit their benchmarks and improve patient care? Join our panelists to learn about the strategies, technologies, and data-driven techniques required to achieve truly value-based practice transformation.

12:00pm - 1:15pm

Networking Luncheon

1:15pm - 2:00pm

Afternoon Keynote: Ensuring the Long-term Success of a Quality Measurement Program

A significant challenge of value-based care is demonstrating continued quality improvement year over year. Without a quality measurement program that evolves alongside the work of providers to treat patient populations with increasing levels of effectiveness, organizations are unlikely to demonstrate continued improvements to clinical efficiency and patient outcomes. In his keynote, National Quality Forum’s Jason Goldwater will discuss the need for dynamic, longitudinal measurement of quality to sustain success in value-based care in to the future.

Jason C. Goldwater, MPA, MA, Senior Director, National Quality Forum

2:00pm - 2:45pm

Panel Discussion: Conquering Financial Risk through Patient Engagement and Coordinated Care

Risk-based financial models require organizations to take a new approach to keeping patients engaged, informed, and able to help providers keep unnecessary costs low. In this panel, attendees will hear about best practices and successful strategies for deploying the programs and implementing the technologies to support these patient-centered care processes.

Deborah Blazey-Martin, MD, MPH, Chief, Internal Medicine and Adult Primary Care, Tufts Medical Center
Terrie Enis, PT, MSPT, Director of Rehabilitation Services and The Dr. Robert C. Cantu Concussion Center, Emerson Hospital

2:45pm - 3:15pm

Networking Break in the Exhibit Hall

3:15pm - 4:00pm

Panel Discussion: Thriving in the Value-Based Care Environment as an Independent Physician

Mergers, acquisitions, and physician employment may be on the rise, but small groups and independent doctors still play a vital role in delivering primary and specialty care. How can physicians develop the skills they need to shoulder financial risk, work with business partners, and engage in population health management while maintaining their self-sufficiency?

Sarika Aggarwal, MD, MHCM, Chief Medical Officer, Beth Israel Deaconess Care Organization
Shawn Purifoy, MD, Medical Director, Aledade Arkansas ACO, Aledade

4:00pm - 4:45pm

Day One Closing Keynote: Extending Value to the Post-Acute Extreme of the Care Continuum

Reducing the risk of patient populations is a cornerstone of effective value-based care. Providers must minimize unnecessary care and provide care management throughout the patient experience. The post-acute and long-term care space presents growing opportunities for care teams to ensure that patients remain comfortable in their homes and avoid appearing in the emergency room in preventable circumstances. In the closing keynote, Partners Continuing Care CIO John Campbell will detail how the post-acute division of Partners Healthcare System combines care coordination and information sharing across post-acute care settings to support value-based care.

John Campbell, CHCIO, Chief Information Officer, Partners Continuing Care

5:00pm - 6:00pm

Networking Reception in the Exhibit Hall

November 17

8:30am - 9:00am

Networking Breakfast for Day 2

9:00am - 11:30am

Choose one of three Workshops (There will be a 15 minute networking break from 10:15-10:30)

-

Workshop A: Negotiating Value-Based Contracts for New Telehealth Services

Telehealth and telemedicine programs are quickly becoming a staple of every health system’s service platform. But because of wildly varying state parity laws and complex Medicare and Medicaid guidelines, hospitals often don’t know whether they’ll be reimbursed for those services. Faced with this uncertainty, health system administrators are reaching out to the payer market to negotiate their own value-based contracts. And in many cases, health plans and private payers are quite willing to strike a deal – if a service has proven its value.

This workshop will bring together telehealth and telemedicine experts, health system administrators and payer representatives to discuss how these new contracts are negotiated. How does a hospital go about proving a new telehealth platform’s value? What does a payer look for to be convinced that such a service should be reimbursed? And who sets the rates?

Andrew Solomon, MPH, Project Manager, Northeast Telehealth Resource Center, MCD Public Health
Alexis Bortniker, Senior Counsel, Foley & Lardner LLP
Carla Bettano, Telederm Program Manager, Consultant, Edward M. Kennedy Community Health Center

Workshop B: Understanding HL7 FHIR: Big Data Workshop and Application Showcase

HL7 FHIR has quickly become one of the hottest acronyms in the health IT lexicon…but what is it, how is it used, and what can it actually do to improve the quality of care?

Join application developers and representatives from HL7 International to learn about FHIR and see the standard in action during our Demo Day Showcase. No coding skills are required – just an interest in how to leverage this new generation of health IT tools to access actionable insights into key clinical and financial challenges.

After an introduction to FHIR by Micky Tripathi, Project Manager of the Argonaut Project, and short demonstrations of each application, attendees will have an opportunity to explore the use cases in detail with small breakout discussion groups.

Workshop C: Strategies for Extending Value Beyond Primary Care

Explore how post-acute and sub-acute partnerships can enhance a healthcare organization’s value-based purchasing strategy. With bundled payment models and ACOs putting provider revenue at risk even after a patient leaves their direct care, organizations should collaborate with post-acute and sub-acute care facilities to control costs and maintain high-quality care.
