Mental health remains one of the most under-resourced sectors of global health, despite the enormous burden it places on individuals and societies. Digital technologies, and more recently, large language models (LLMs), have been proposed as scalable solutions to bridge gaps in mental health care delivery. LLMs, which process and generate language, are particularly suited to mental health due to the centrality of language in diagnosis, monitoring and therapeutic interventions. However, the integration of these models into clinical settings is not without risk. Technical, ethical and sociocultural considerations must all be addressed. A sociocultural–technical framework offers a promising path forward, seeking to balance innovation with equity and inclusion.
Unlocking Potential: Where LLMs Can Transform Mental Health Care
The promise of LLMs in mental health lies in their capacity to interpret unstructured text data, such as clinical notes and transcripts, which are foundational to psychiatric care. These models can potentially assist in early diagnosis, patient monitoring, personalised interventions and even therapeutic support. Given that mental health conditions are often identified and treated through communication, LLMs are uniquely placed to enhance linguistic analysis and prediction within clinical workflows.
LLMs operate as highly advanced pattern-recognition tools, trained on massive datasets composed of textual inputs. When applied to mental health, they could automate routine tasks such as summarising clinical notes or identifying linguistic markers of psychiatric symptoms. Furthermore, LLMs may provide empathetic responses and support clinical decision-making. Nonetheless, their effectiveness hinges on the robustness and representativeness of the data used to train them, underscoring the importance of building diverse and comprehensive training datasets. While some studies have demonstrated encouraging results in using LLMs for diagnosis and intervention support, these successes must be scaled responsibly and equitably.

Navigating Limitations: Bias, Access and Transparency
Despite their potential, LLMs present substantial challenges that hinder clinical deployment. A primary concern is the lack of transparency in training datasets, which are often composed predominantly of English-language and Western-centric content. This bias compromises the accuracy and reliability of LLMs in multilingual or culturally diverse settings. The authors of the framework suggest a solution grounded in transparency and equity. Regular audits of data provenance, diversity tracking and ethical usage monitoring should become standard. Moreover, developing models through federated learning, where data remains decentralised, can enhance privacy while ensuring broader participation. Framework in Action: Building Inclusive, Ethical Ecosystems for LLM Deployment
To operationalise LLMs in mental health responsibly, the framework proposes five strategic areas: constructing a global clinical repository, fostering ethical usage, redefining diagnostic structures, respecting cultural variance and promoting digital inclusivity. A global, multimodal biobank encompassing clinical notes, research data, behavioural signatures and biomarkers is foundational. Such a repository must be governed by an independent body with clear guidelines on privacy and data use, including the adoption of federated systems and strict cybersecurity protocols.
Equally important is the design of systems that enable ethical application of LLMs in clinical practice. These systems should facilitate shared decision-making, support therapy-related activities and ensure sensitive patient data remains protected. While LLMs can enhance patient-clinician communication and empower self-management, safeguards must be in place to prevent misuse or over-reliance. Ethical ecosystems should also consider the life cycle of these models, including their environmental impact and hardware requirements.
A critical component of the framework is the re-examination of psychiatric diagnostics. Existing tools, such as the DSM-5, lack universal reliability and fail to reflect the complex spectrum of mental health experiences. LLMs, through their ability to synthesise vast and varied data, could aid in the identification of nuanced diagnostic markers and stratify mental health conditions with greater precision. Nevertheless, LLMs should function as supportive aids within interpretable systems, ensuring clinicians remain central to the diagnostic process.
The framework further emphasises the importance of acknowledging and adapting to cultural and linguistic differences in mental health expression. Transparent and inclusive dataset curation, coupled with flexible model design, can help LLMs better serve diverse populations. Engaging individuals with lived experience and domain experts is crucial to ensure that the development and deployment of these tools reflect real-world needs and values.
Finally, digital inclusivity must be a guiding principle. As internet access and digital tools become more widespread, disparities still exist along lines of race, gender, education and income. Promoting equitable access includes not only infrastructural development but also improving digital literacy. The concept of the digital navigator—community members trained to support digital engagement in health contexts—illustrates one way to bridge the digital divide and ensure new technologies benefit all communities.
The integration of LLMs into mental health care offers promising avenues for transforming research and clinical practice. However, this transformation must be undertaken with caution, clarity and inclusivity. By adopting a sociocultural–technical framework, stakeholders can develop and deploy LLMs in ways that uphold ethical standards, enhance cultural sensitivity and ensure equitable access. Establishing global repositories, fostering digital literacy and redesigning diagnostic tools are all critical components of this process. The involvement of governments and health systems in shaping policy and accountability structures will be vital to sustainable implementation. Through collective effort, LLMs can become tools not just of innovation, but of justice in global mental health care.
